Patient Assistance Initiatives
The URL Pharma COLCRYS® Patient Assistance Program offers in-need patients an opportunity to receive COLCRYS at reduced cost. Patients with a valid COLCRYS prescription who are legal US residents and who are either uninsured or covered only by Medicare Part D may be eligible for the URL Pharma COLCRYS Patient Assistance Program.

Proof of household income will be required as part of the application process. Patients with Medicare Part D prescription coverage will be asked to attest that they will not claim co-payments on their Medicare plan. Patients are eligible to receive COLCRYS through the program for a period of one year from the date of initial enrollment, and will need to reapply to the program annually.

Reimbursement Hotline – specialized assistance to facilitate reimbursement.
The URL Pharma Reimbursement Hotline offers advice and guidance to patients, caregivers, physicians, and pharmacists needing assistance securing patient access to COLCRYS. Staffed by Pharmacy Healthcare Solutions, Inc., the reimbursement hotline can be reached at 1.866.311.2294 from 8:00 AM to 5:00 PM EST, Monday through Friday, or via e-mail at urlpharma@phsirx.com.
COLCRYS co-pay reduction programs.
Patients who do not qualify for the Patient Assistance Program can benefit from COLCRYS co-pay assistance. Many patients can have their co-pay reduced to $25 (some restrictions apply). Please click on the "Download a coupon button" to view the terms and conditions and to download a reusable coupon.
